Cell deconvolution and differential gene expression analyses were performed to characterize transcriptomic signatures of healthy pulp (HL), reversible pulpitis (RP), and irreversible pulpitis (IP), with specific attention to coronal regions adjacent to carious lesions.<h4>Results</h4>Clinically diagnosed IP samples, presenting with deep caries, percussion sensitivity, and lingering cold pain, shared molecular features with RP, including a similar immune-fibroblast ratio and activation of TLR4 and neuroinflammation pathways. This evidence concerns the gene TLR4 and incontinentia pigmenti.
