The 2021 AHA/ASA Guideline for the Prevention of Stroke states that for high-risk ischemic stroke patients whose LDL-C levels remain above 70 mg/dL despite receiving maximum tolerated statin therapy combined with ezetimibe, the use of PCSK9 inhibitors for secondary prevention is considered reasonable (Kleindorfer et al., 2021).
