This group comprises all patients who were detected solely on the basis of a positive DRE and who, in the absence of DRE at that time point, would have escaped further diagnostic work-up within the screening pathway. Among DRE-suspicious patients with PSA < 4 ng/mL, the overall PCa detection rate was 67.9% (19/28), with clinically significant cancer (ISUP ≥ 2) detected in 35.7% (10/28) of cases.
