INS and diabetes mellitus: Fasting and postprandial blood glucose levels were 187 mg/dL and 208 mg/dL, respectively, with an HbA1c of 7.3%, consistent with newly diagnosed diabetes mellitus; the diagnosis, made as a chance discovery in an asymptomatic patient, led to initiation of subcutaneous regular insulin at 1 IU/kg/day in divided doses, with subsequent titration guided by capillary glucose monitoring.
