The Prato Polymyxin Consensus states that in patients with renal insufficiency who are on intermittent hemodialysis, the following dosing regimen should be used: on non-dialysis days, a 130 mg mucin-activated base (CBA)/day dose of colistin sodium mesylate should be given; on dialysis days, a supplemental dose of either 40 mg of colistin sodium mesylate or 50 mg of CBA should be given; and, if possible, a supplement to the baseline (non-dialysis) daily dose should be administered with the next regular dose at the end of the day.
