ACE and hypertensive disorder: Current recommendations for the treatment of orthostatic hypertension focus on agents that are not considered first line for seated hypertension (that is, thiazide diuretics, dihydropyridine calcium channel blockers, or angiotensin converting enzyme inhibitors/angiotensin receptor blockers).6 Whether more intensive treatments generally used for seated hypertension might be efficacious for orthostatic hypertension is unknown, but it could have implications for the formulation of treatment strategies to tackle this emerging hypertensive phenotype.
