For this entity, which is now defined as DLBCL/HGBL with MYC and BCL2 rearrangements in the World Health Organization (WHO) 5th edition [14], consideration of intensive first-line immunochemotherapy regimens, such as dose-adjusted etoposide, prednisone, vincristine, cyclophosphamide, doxorubicin, and rituximab (DA-EPOCH-R), is recommended in NCCN guidelines [13], although this is supported largely by retrospective data that suggest a benefit to these regimens over R-CHOP [15,16].
