INS and myocardial infarction: A 68-year-old male with T2DM, obesity, and acute myocardial infarction is on Metformin, Insulin, Clopidogrel, Spironolactone, Furosemide, Lisinopril, Carvedilol, Rosuvastatin, presented with malaise, myalgia, chest pain, dry cough, dyspnea, and dark-colored urine. His CK level was 1,002, LDH of 559, and creatinine was 689, and he survived the episode.
