Most disagreements arose concerning three START criteria involving the use of acetylcholinesterase inhibitors for mild-to-moderate Alzheimer’s dementia or Lewy Body dementia (C3), the use of vitamin D and calcium supplements in patients with known osteoporosis and/or previous fragility fractures (E3), and use of bone anti-resorptive or anabolic therapy (e.g., bisphosphonate, strontium ranelate, teriparatide, and denosumab) in patients with documented osteoporosis where no pharmacological or clinical contraindications exist (E4).
