Another important question is whether omitting ALND would preclude eligibility for treatment with a cyclin-dependent kinase (CDK) 4/6 inhibitor on the basis of the total number of positive LNs.31 Of the 80 patients with HR+/HER2– undergoing ALND, five (6%) of 80 had ypN2 disease, but only one patient would have not been eligible to receive a CDK 4/6 inhibitor on the basis of other high-risk features at presentation (tumor size or grade).
