Classified as intermediate-risk are microscopic invasion of tumor in the perithyroidal soft tissues, iodine-avid metastatic foci in the neck, aggressive histology, papillary thyroid cancer with vascular invasion, clinical N1 or >5 pathologic N1 (<3 cm) or a multifocal papillary microcarcinoma with extrathyroidal extension and/or BRAF V600E mutation.
