The most recent 2023 National Comprehensive Cancer Network (NCCN) guidelines recommend the following as patient criteria for discontinuation: an age >18 years, chronic-phase CML (CP-CML) with no prior history of accelerated-phase CML (AP-CML) or blast-phase CML (BP-CML), being on approved TKI therapy for at least 3 years with prior evidence of a quantifiable BCR-ABL1 transcript and MR4 (BCR-ABL PCR ≤ 0.01% IS) for ≥2 years with reliable access to qPCR testing with a sensitivity of detection of at least MR4.5 (BCR-ABL PCR ≤ 0.0032% IS) that provides results within 2 weeks [14]. This evidence concerns the gene ABL1 and chronic myelogenous leukemia, BCR-ABL1 positive.
