The Italian Association of Medical Oncology [91], the American Society of Clinical Oncology [15,93], the National Comprehensive Cancer Network [92], the European Society of Medical Oncology [17] and the International Initiative on Thrombosis and Cancer [16] guidelines strongly suggest the use of venous thromboembolism pharmacological prophylaxis with low-molecular-weight heparin, unfractionated heparin, or fondaparinux (factor Xa inhibitor) in all hospitalized cancer patients (Table 3).
