The typical findings of BAL in pulmonary sarcoidosis include a normal or mildly elevated total cell count with lymphocytosis(CD4+/CD8+ ratio > 3.5 is high specific), a normal percentage of eosinophils and neutrophils, and an absence of plasma cells and foamy alveolar macrophages; active sarcoidosis tends to show higher lymphocyte counts than inactive sarcoidosis [35].
