The reduction in DHEAS demonstrated here raises an intriguing hypothesis whereby SGLT‐2 inhibitors, by virtue of their special capacity to lower body weight and enhance glucose uptake, would decrease hyperinsulinemia, along with a reduction in DHEAS leading to a reduction in free testosterone, which in turn would improve glucose utilization, forming a basis for breaking the vicious cycle of hyperinsulinemia and hyperandrogenism, which is the foundation of PCOS. The gene discussed is SLC5A2; the disease is polycystic ovary syndrome.
