Risk-reducing bilateral salpingo-oophorectomy should be offered from age 35 years and 40 years in BRCA1 and BRCA2 carriers, respectively.7 If there are very early-onset ovarian cancers (younger than age 40 years in BRCA1 carriers and younger than age 45 years in BRCA2 carriers) in the individual's family, these ages may be reduced pragmatically to 5 years younger than the age of onset of the earliest ovarian cancer (but not breast cancer) in the family.
