AGT and type 1 diabetes mellitus: Although an acute eGFR decline may raise safety concerns among clinicians, the post hoc analysis of the Reduction of Endpoints in Non–Insulin-Dependent Diabetes Mellitus with the Angiotensin II Antagonist Losartan (RENAAL) trial showed that losartan, an angiotensin II receptor blocker, induced a significantly greater acute eGFR decline during the first 3 months of treatment compared with the placebo but a significantly slower long-term eGFR decline in follow-up.35
