The factors available at the time of hospital discharge that were significantly associated with increased one-year mortality in the univariable analysis (with P < 0.05) were older age, not living at home, home visits by nurses, dementia, active cancer, liver cirrhosis, hyponatremia, lower hemoglobin and albumin levels than at admission, and higher scores in the early simplified HOSPITAL score with laboratory results at admission (Table 2).
