Severe AS induces a very turbulent blood flow at the vicinity of the aortic valve, causing high shear stress that promotes deployment and cleavage of von Willebrand factor (VWF)'s high-molecular-weight (HMW)-multimers results in an increase of CT-ADP.23Moreover, a significant PVL (>mild) can also induce HMW-multimer defects because of its turbulent flow.
