In the absence of randomized studies comparing BCR::ABL1 TKIs in adults withPh-positive ALL, a recent study in a pediatric population of Ph-positive ALLprovides some guidance.18 In this phase III trial, 189 patients (up to the age of18 years) were randomized to receive chemotherapy with imatinib300 mg/m2/day or dasatinib 80 mg/m2/day, a dose ofdasatinib associated with enhanced central nervous system (CNS)penetration.19 Dasatinib resulted in higher rates of 4-year EFS (71%versus 48.9%; p = 0.005) and 4-year OS(88.4% versus 69.2%; p = 0.04) compared withimatinib. The gene discussed is BCR; the disease is acute lymphoblastic leukemia.
