However, in a study of 47 HCC patients receiving a PD-1 inhibitor in combination with antiangiogenic therapy and radiotherapy, patients with low PD-L1+CTCs at baseline had a higher ORR (56.5% vs. 16.7%, p = 0.007) and longer OS (not reached vs. 10.8 months, p = 0.001) than those with high PD-L1+CTCs (83), indicating that CTC is still a controversial biomarker for predicting the treatment response to immunotherapy in HCC.
