Recent evidence suggests that albumin, one of the best characterized markers of hepatic progenitor cells, may be a good biomarker of iCCA but does not distinguish from other primary intrahepatic malignancies such as HCCs.31 Novel characteristic histologic features of iCCA may also be useful, including the recently described cholangiolar pattern that is comprised of well-formed ducts with angular profiles that mimic antler horns.31 Lastly, presence of alterations in FGFR2, IDH1, and BAP-1 oncogenes on biomarker testing, especially in some combination, can strongly support iCCA diagnosis.32 Here, BAP1 is linked to infantile convulsions and choreoathetosis.
