A uremic myopathy [95,96] and hormonal derangements (i.e., decreased serum testosterone levels [97] or resistance to the actions of growth hormone [98,99] or insulin-like growth factor-I (IGF-I) [100,101], hyperglucagonemia [102,103], and hypothyroidism) [104,105] may also contribute to these disorders.
