LACTB and ventilator-associated pneumonia: Here, we describe a case of bacteremia and ventilator-associated pneumonia (VAP) due to an extensively drug-resistant (XDR) Klebsiella pneumoniae strain resistant to novel beta-lactam/beta-lactamase inhibitors and to cefiderocol that was successfully treated in a critically ill patient with a combination of a high-dose continuous infusion of meropenem plus fosfomycin guided by real-time therapeutic drug monitoring.
