Pharmacist-led interventions have unique advantages, as pharmacists are experts in the medications that achieve core targets of primary prevention, including blood pressure, glucose, and lipid lowering; pharmacists are well-positioned to counsel patients about sodium-glucose cotransporter-2 (SGLT-2) inhibitors, one of the most promising recent medications for the prevention of heart failure (17); and pharmacists may, under collaborative practice agreements with supervising physicians, directly prescribe and titrate medications for patients.
