POMC and Hyperkalemia: Based on the biochemical features of hyponatraemia, hyperkalaemia, hypoglycaemia, low cortisol, low aldosterone, high ACTH in the presence of low 17-hydroxyprogesterone and dehydroepiandrosterone sulphate levels, he was diagnosed to have primary adrenal insufficiency due to a defect in the initial steps of adrenal hormone biosynthesis or congenital adrenal hypoplasia.
