From among these, the stepwise model selection and random forest algorithm identified serum M-protein concentration, involved to uninvolved sFLC ratio, and BMPC percentage as the most relevant factors predicting progression to MM, and based on Youden’s Index in the ROC analyses, the optimal cut-offs for the risk factors were 1.9 g/dL for serum M-protein (specificity 60% and sensitivity 71%), 19.3 for the involved to uninvolved sFLC ratio (specificity 79% and sensitivity 55%), and 16.4% for the BMPC infiltration (specificity 55% and sensitivity 75%) as shown in Table 3.
