CYP3A4 and rhabdomyolysis: This inhibition in conjunction with competitive CYP3A4 inhibition (due to increased concentration of both drugs and metabolites) results in increased simvastatin levels and rhabdomyolysis.11 While there are a few case reports of abiraterone solely inducing rhabdomyolysis, our patient tolerated abiraterone and simvastatin until the onset of biliary obstruction, which precipitated the rhabdomyolysis.
