Genetic testing at the time of diagnosis of cancer (referred to by some as “mainstreaming”57) has several advantages.20 It identifies those who may benefit from poly (ADP-ribose) polymerase inhibitors for treatment of breast, ovarian, prostate, and other cancers58-61; checkpoint blockade immunotherapy for those with mismatch repair deficiency62; or targeted treatment of those with inherited mutations in MTOR,63,64RET,65Hedgehog,66 or other pathways.
