Evolocumab treatment significantly reduced the risk of ischemic stroke (HR, 0.75 [95% CI, 0.62–0.92]), without a significant effect on hemorrhagic stroke (HR, 1.16 [95% CI, 0.68–1.98]).10 Although PCSK9 inhibition may lower LDL-C to levels far below those achieved with statins alone, FOURIER found that lower achieved LDL-C did not increase the risk of hemorrhagic stroke, even when LDL-C levels were <0.5 mmol/L (20 mg/dL).18 The gene discussed is PCSK9; the disease is hemorrhagic stroke.
