For alcohol, randomized controlled trials demonstrate that nalmefene, a combined mu-opioid receptor antagonist and partial kappa-opioid receptor agonist, is effective in reducing a number of alcohol use outcomes, compared with placebo, in individuals with AUD (see Mann et al. for a review).21 Older studies have also evaluated nalmefene for PTSD, with some indication that nalmefene reduces emotional numbing, nightmares, flashbacks, intrusive thoughts, and other PTSD-associated symptoms.22 However, to date, no studies have examined nalmefene for comorbid AUD and PTSD.
