Hoorn EJ et al. postulated a new mechanistic pathway of hyperkalemia in transplant recipients by demonstrating that tacrolimus activates the thiazide-sensitive sodium-chloride cotransporter (NCC) in the distal convoluted tubule (DCT) leading to hyperkalemia and hypertension similar to the ones that occur in Gordon syndrome (13).
