P2RY12 and diabetes mellitus: On the basis of the available evidence, we recommend consistent and highly effective platelet inhibition with the newer, more potent P2Y12 inhibitors for patients with ACS and concomitant diabetes, given their high baseline risk profile; therefore, DAPT with aspirin plus prasugrel or ticagrelor should be the first-line antiplatelet strategy up to 1 year in this setting, especially in patients without high bleeding risk (Table 3).
