The median CRP test valuewas lower than 20 mg/L, a number that has been used by some authors to suggest adiagnosis of pyelonephritis.15,21 In the urinalysis, a positive nitrite testwas highly specific and had a high PPV; therefore, if initial screening reveals apositive nitrite test, antimicrobial agents should be started immediately aftercollecting samples for quantitative urine cultures.
