PDE5A and erectile dysfunction: The identification of more extensive small-fibre damage using CCM may allow us to identify those patients with erectile dysfunction who are less likely to respond to conventional therapies such as PDE5 inhibitors, and who should therefore be considered for daily or higher doses of PDE5 inhibitors, combination therapies or, indeed, alternative therapies such as intraurethral alprostadil or a penile prosthesis [33, 34].
