The patient's history of having a papillary thyroid carcinoma (noted to have severely atypical nuclear features) coupled with the above immunohistochemistry and the laboratory findings of an extremely elevated thyroglobulin level would be most indicative of a metastatic papillary thyroid carcinoma with anaplastic transformation (multifocal in this case). This evidence concerns the gene TG and thyroid gland papillary carcinoma.
