A randomized, open-label, 24-week trial, comparing biphasic insulin aspart 70/30 od or bid against exenatide (on a background of metformin and SU), showed that glycaemic control was better with biphasic insulin (in terms of HbA1c and higher proportion of patients reaching target HbA1c of <7% or <6.5%); however, insulin use was associated with higher risk of hypoglycaemia and weight gain [25].
