The WHO guidelines recommend that HIV-infected adults with CD4 counts <100 cells/μL and CrAg detected in serum or plasma receive a tapered course of fluconazole (800 mg daily for 2 weeks, followed by 400 mg daily for 8 weeks and then 200 mg daily pending immune reconstitution) as long as they have no signs or symptoms of meningitis (in which case they should receive a lumbar puncture to evaluate for meningitis).
