The Kidney Disease Outcomes Quality Initiative (KDOQI) guidelines recommend that patients with diabetes should be screened by simultaneously measuring the urinary albumin-to-creatinine ratio (ACR) and estimated glomerular filtration rate (eGFR).12 The KDOQI also mentioned special circumstances in which to suspect nondiabetic renal disease, such as in patients with diabetes and a low eGFR who do not have microalbuminuria or macroalbuminuria or other microvascular complications, including diabetic retinopathy.
