In a large retrospective cohort study of paracentesis on cirrhotic patients, major complications were associated with therapeutic procedures, not diagnostic ones.19 Terlipressin plus diuretic and albumin seemed effective in patients with refractory ascites in a multicentric study.23 Therefore we suggested that, instead of therapeutic paracentesis, less aggressive therapy as terlipressin could be used for patients with extremely low levels of fibrinogen to minimized bleeding complications.
