The thrombolytic, tPA, has become essentially synonymous with thrombolysis but its use, or that of one of its longer half-life derivates, has been declining due to its inadequate efficacy in AMI, incompatibility with PCI, limited efficacy and risk of ICH in ischemic stroke, and too high a bleeding risk for most patients with venous thromboembolism.
