Regarding this phenomenon, our explanations are as follows: (1) the neuroprotective role of TNF-α inhibitors cannot been fulfilled within a short period of time; (2) we conservatively speculated that TNF-α-related inflammation and neurotoxicity are not the major pathophysiological mechanisms of sciatica; thus, TNF-α inhibitors might not exhibit clinical value at long-term follow-up; (3) the bias of drug administration approaches might be involved in this result.
