Medical exclusions included active and inadequately treated thyroid disease (n = 6), neurological disease (n = 6), inflammatory disease (n = 4), C-reactive protein levels 2 to 4 times normal (n = 3), severe anemia (n = 2), uncontrolled diabetes (n = 2), cardiac disease (n = 2), renal disease (n = 2), breast cancer post-treatment (n = 1) and liver disease (n = 1).
